The Bloom Wellness Membership is a voluntary self-pay psychiatric care program for individuals who do not have insurance or who prefer not to use insurance for psychiatric services. Members receive the services included in their selected plan for a predictable monthly fee.
How It Works
Step 1: Schedule Your Initial Psychiatric Evaluation
All new patients must complete an initial psychiatric evaluation before enrolling.
The evaluation fee is $350 and is billed separately.
Step 2: Join the Bloom Wellness Membership
If you enroll in a membership within 14 days, the $350 evaluation fee will be credited toward your first membership payment, subject to the Membership Agreement.

Comprehensive psychiatric assessment, 60 to 90 minutes
Review of your medical, psychiatric, family, and social history
Opportunity to discuss your goals and ask questions
Individualized treatment plan
Personalized treatment recommendations
Diagnostic evaluation
Medication review
Best for: Patients who are stable on their medications and generally require routine monthly follow-up.

$199 per Month
Designed for patients who are stable on their current treatment plan and typically require one follow-up appointment each month.

Membership does not guarantee that a controlled medication will be prescribed. All medication decisions are based on clinical appropriateness, safety, monitoring requirements, and applicable laws and regulations.

Services Not Included
Membership Term
The Bloom Wellness Membership is a six-month membership commitment designed to promote continuity of care and support your long-term treatment goals.
Membership begins on your enrollment date and remains active for six months. Membership fees are conveniently billed monthly throughout the six-month membership term.
At the end of each six-month term, your membership will automatically renew for another six-month period unless canceled according to the cancellation policy.
30-Day Cancellation Policy
Members who do not wish to renew must provide at least 30 days' written notice before the end of their current six-month membership term.
Your membership and benefits will remain active through the end of your current membership term.
Cancellation requests received during an active six-month term will become effective at the conclusion of that term.
Membership benefits are non-transferable and apply only to the enrolled patient.
Blooming Rose Psychiatry & Family Practice may update membership pricing, benefits, or terms at the time of renewal. Members will receive advance notice of applicable changes before their next membership term.
Insurance or Membership
Blooming Rose Psychiatry & Family Practice offers two payment options for psychiatric care.
Insurance
Patients must choose either insurance billing or the Bloom Wellness Membership for psychiatric services. Membership services will not be submitted to insurance, and insurance and membership benefits cannot be combined for the same service.
Bloom Wellness Membership
Patients who do not have insurance may enroll in the Bloom Wellness Membership and receive the psychiatric services included in their selected membership for one predictable monthly fee.
Patients who have insurance but choose the self-pay membership option should understand that Blooming Rose Psychiatry & Family Practice will not submit membership services to their insurance.
Patients must choose one payment option for their psychiatric services. Insurance and the Bloom Wellness Membership cannot be combined for the same services.

Important Membership Notice
The Bloom Wellness Membership is a voluntary self-pay program offered by Blooming Rose Psychiatry & Family Practice, a DBA of Blooming Rose Health Group, PLLC.
The membership is not health insurance and is not a substitute for health insurance. Membership covers only the services specifically identified in the patient's Membership Agreement. Outside medical services, medications, laboratory charges, emergency services, hospital services, and other excluded services remain the patient's financial responsibility.
All membership services, fees, renewal terms, cancellation requirements, and exclusions are subject to the signed Bloom Wellness Membership Agreement.
